Pre-Session Paperwork

After booking your session, please fill out this Client Information Form as soon as possible. It will help me get to know you and your family a little better, and will also help me personalize your portrait session.

Once you have submitted the Client Information Form, please review this Portrait Session Agreement and Model Release, then print out two copies. Sign one of them and return it to me at the address below, and then keep one for your records. If you do not have access to a printer, please review the Agreement and I will bring a copy to your session for you to sign.

Meet Jen

Here are a few things you might want to know about me...
• I'm a totally self-taught photographer. In college I majored in Education and French. Seriously.
• I'm the mother of two fun and loveable sons, ages thirteen and nine. You will probably see lots of pictures of them here.
• I also have the best husband a girl could ever ask for. You won't see many pictures of him, because he's camera shy.
• We have a big, crazy dog running through our little, bitty house. He's a Lab mix named Otto. Did I mention that he's crazy? Oh, and we decided to add a new puppy to the mix, just to keep things interesting. She's a husky named Ursa, and she's already running the house.
• During the school year I work as a tutor for the Fulton City School District. This is to satisfy my mother and let her know that yes, I am using that degree she paid for.
• I love people and I love to talk. If you call me, there is a good chance that I will talk your ear off. Don't be afraid to tell me to stop.
• I'm a list-maker. I honestly could not survive without my to-do lists.
• I really like bullet points.
• I've been doing this photography thing for over five years now, and I still get nervous and excited before every shoot!
• My true love, my passion, the thing that gets me out of bed every morning? It's the pictures. Pictures are my life.
